New Client Intake (includes office policies)

If you or your child is a new client, you may wish to fill out this form for adults or this form for minors in advance and bring it to your first session. Physician's Prescription Form

Some insurance companies cover massage therapy for specific conditions. I do not bill insurance directly but can give you receipts to submit to your insurer. Your insurer will expect you to have a prescription for massage from your physician. To give you a prescription, your physician can complete this simple form.  

Physician's Permission Form

For certain conditions I will need permission from your physician to work with you or to provide deeper work. Usually I can figure this out when we speak on the phone before our first session. This is a simple form your physician can fill out to give clearance for me to work.
